The dissemination of any patient information is limited to only those individuals who have a preapproved use for the information, and only information necessary for that specific use is released. To reinforce our security systems, we follow established guidelines for masking patient identity in electronic records and other working documents where the identity of the patient is not necessary.
You have the right to expect that all records pertaining to your care will be treated as confidential to the extent permitted by law. You also have the right to expect that information about you receives fair treatment by those who must use it in order to provide you with services. Please note that Total Health Concepts has internal penalties for misuse of patient information, as well as confidentiality controls on our electronic transaction systems.
