How to Become a Medical Marijuana Patient

THE PROCESS

1) MEDICAL RECORDS & ACCEPTED I.D.’S The patient must request their physician or specialist to send their medical records to that patient.  These records will show forth a condition that is on the list of ailments for which someone can be referred to medical marijuana.  Current accepted conditions include:

Cancer, Glaucoma, HIV positive, AIDS (OR a chronic or debilitating disease or medical condition that produces, for this patient, one or more of the following and which, in the physician’s professional opinion, may be alleviated by the medical use of marijuana), Cachexia, Severe Pain (Doctor’s Discretion/Etiology of Pain), Severe Nausea, Seizures and Muscle Spasms

PATIENTS AND CAREGIVER’S PROOF OF IDENTITY AND PROOF OF RESIDENCY IN COLORADO

At least 1 of the following:

Colorado Driver’s License

Colorado I.D.

Temporary Colorado Driver’s License

Temporary Colorado I.D.

Or at least 2 of the following:

Out of State Driver’s License

Out of State I.D.

Passport, Military I.D., Tribal I.D.

And a Minimum of 1 from the group below:

Colo Work Identification/paycheck stub/W-2,

Utility Bill, Medical/Insurance Bill, or Cable Bill

The above items must show a Colorado residence.

At least one of these documents must show the applicant’s date of birth.


2) FIND A DOCTOR/GATHER FEES Find a doctor that refers medical marijuana.  Here at Total Health Concepts, we can help you locate a doctor, set up an appointment and send off the appropriate forms to the State Registry.  Remember, the state currently charges a fee of $90 EVERY time you renew your paperwork.  Generally, the patient will use the referral forms as a temporary license for about eight months and THEN they will receive a tri-folded red Medical Marijuana card which is good one year from the process date.

3)  DETERMINE YOUR GROWER The Registry gives patients several options in this regard.

a. Self (You can grow all six plants yourself)

b. Care-giver (You can select an individual as your registered care-giver.  They are limited to five patients they can give care to.

c. Medical Marijuana Center (You designate a MMJ Center/dispensary to grow all six plants for you).

**Total Health Concepts offers a free eighth ounce EVERY month for free as well as 10% of the monies spent given back to the patient in the form of a store credit at the beginning of every month for the previous month.**

d. Self and Medical Marijuana Center (You can grow three plants and the MMJ Center is allowed to grow the other three).

Bottom line, pick a MMJ Center that is convenient (close in proximity), offers real perks for that center being allowed to grow for you and knows the specifics of the industry/products.
