First, you will be greeted with one of Total Health Concept’s staff who is more than happy to understand the patients ailments and guide them in choosing the best medicine for their needs. Before showing medicine, we ask our patients for copies of all required paperwork (a current physician certification form, notarized application form, a legitimate local I.D. and the green certified mailing receipt to show forth payment was made to the Registry) OR an up-to-date red, state assigned MMJ card.
THEN….
You will be asked to fill out our membership packet and HIPAA form (this form protects your privacy and ONLY allows for the portion of your medical records concerning medical marijuana to be disclosed).
